Harry M. Vars, PhD was a visionary leader of the clinical nutrition community, who in his work and in his professional life, exhibited the qualities of passion, evidence, and compassion that are needed by both individual members and our society to assure that every patient receives optimal nutrition care.

While feasible and well-tolerated, a 1-year course of standard and immune-enhancing oral formulas resulted in no differential effects on nutrition or immune-status parameters in a diverse sample of asymptomatic HIV-infected persons.

Treatment with LCT vs. MCT/LCT emulsions produce an increase in linoleic acid (18:2n6) and a decrease in arachidonic acid (20:4n6) in phospholipids’ fatty acids that are suggestive of an inhibition of {Delta}-5-desaturase.

Forty-six methods obtained from the literature for estimating energy expenditure in thermal injury were evaluated for bias and precision in 24 thermally injured adults requiring specialized nutrition support. None of the methods was precise as defined by a confidence interval for error ≤15%. One-third of the methods were biased toward overpredicting measured energy expenditure. In the event indirect calorimetry is not available, the most precise unbiased methods were identified.

Dogs with chronic hepatic denervation and 7 days of treatment with cyclosporine A and prednisone exhibited prolonged hyperglycemia and enhanced net hepatic glucose uptake in response to a mixed meal. These effects are apparently due to immunosuppressant therapy and not to the hepatic denervation. Individuals receiving immunosuppressive therapy are at risk for alterations in glucose disposition in response to mixed meal ingestion.

When fed human milk, fecal short-chain fatty acid (SCFA) concentrations from 8 to 21 days of life were 3-fold lower in extremely preterm infants (n = 10) than in preterm infants born after 33 weeks of gestation (n = 8). A lactose-free formula (n = 10) did not influence the total SCFA excretion but significantly increased the butyric ratio.

Choline deficiency, common in home TPN patients, has long been associated with folate status. Long-term TPN patients with normal folate status have interrelated deficiencies of vitamin B12 and choline, which may be caused by limited intake of cobalamin and choline relative to folate in the TPN infusions.

This is a case report of a 46-year-old man who developed cardiomyopathy apparently due to selenium deficiency while on total parenteral nutrition (TPN). Possible mechanisms, review of literature, and clinical implications are discussed.
